We have designed these policies to help us better serve you and provide the best level of care possible.  If you have any questions regarding our policies, please contact us.

Scheduling Appointments: We strongly recommend scheduling your appointments in advance.  This will give you the opportunity to find the days and times that best fit your needs.


We require a minimum of 24 hour notice to change or cancel a scheduled appointment.  If you fail to come to your appointment or cancel without 24 hour notice you will be charged $50.00.  This charge is your responsibility and will not be billed to your insurance company.  Payment is due before your next appointment.


We accept most forms of private insurance companies. Please call our office to verify our participation with your insurance company.

We are a contracted provider with Medicare.

As a courtesy to our patients, we can bill most insurance companies for you.  However, we cannot guarantee eligibility, extent of coverage, or payment. You are ultimately responsible for any outstanding balance.

Cash Rates:

We do not accept HMO plans, Medical or third-party payors. We do offer a discounted cash rate schedule:

  • Initial Evaluation: $150.00
  • Follow-up Treatments: $90.00
